Use this form to request a cash withdrawal from your FCMM Retirement Plan account. To qualify for this cash withdrawal type, you must have  incurred expenses associated with the birth or legal adoption of a child. The amount requested is limited to $5,000 per child born or legally adopted within the past 12 months. Qualified distributions are penalty-free and [optionally] may be repaid to the Plan. Use this form to request a cash withdrawal from your FCMM Retirement Plan account. This is an in-service distribution limited to those with an immediate and heavy financial need who do not meet the eligibility criteria of other withdrawal options in our Plan. The IRS restricts the availability of funds for hardship withdrawal to the following reasons*: Medical Expenses; Costs directly related to purchase of primary residence; Payment of post-secondary tuition and related fees; Payments necessary to prevent eviction/foreclosure; Burial or funeral expenses; Expenses for repair of damage to principal residence; and Expenses or losses incurred as a result of a FEMA disaster.
